This mixed practice worksheet includes two sections: writing an equation from a given slope and point, and writing an equation from two points by finding the slope first.

This worksheet supports learners who are building fluency with linear equations. Students will:

  • substitute a given point and slope into point-slope form

  • find slope from two points using the slope formula

  • write the final equation using either point

  • work with fractions and negative values (where sign mistakes are most common)
